HOW SERVICES FIT TOGETHER
One resident can need several parts of the service.
A change in walking may also mean a change in transfer assistance. A fall may lead to an equipment review. Pain may reduce exercise participation. A change in cognition may alter how a mobility strategy needs to be delivered.
ACP considers these relationships rather than treating each issue as completely separate.
