WHAT WE ASSESS

Mobility is more than walking speed.

We consider how a person gets in and out of bed, stands from a chair, walks, turns, manages different surfaces and uses mobility equipment. We also consider pain, strength, balance, fatigue, confidence and the level of assistance available.

Walking & gait

Gait assessment, walking practice, mobility aid review and strategies to support safe movement.

Transfers

Assessment of bed, chair and toilet transfers and the level of assistance or equipment required.

Rehabilitation

Individualised rehabilitation following illness, injury, hospitalisation or a change in functional ability.

Functional decline

Identifying meaningful changes in mobility and considering what may be contributing to the change.

Exercise

Strength, balance, mobility and endurance activities linked to functional goals.

Equipment

Review of walking aids, wheelchairs and other equipment that may support mobility and safety.

WHY FUNCTION MATTERS

The outcome is what the person can do in everyday life.

For one person, success may mean walking to the dining room. For another, it may mean standing safely for a transfer or maintaining the ability to participate in a favourite activity. ACP keeps the intervention connected to that real-world outcome.
